A 2024 guideline from the US-based American College of Gastroenterology (ACG) uses the ratio of aspartate aminotransferase (AST) to alanine aminotransferase (ALT) only as one part of an alcohol-associated hepatitis assessment. Raw enzyme values, symptoms, exposure history and other tests determine what follow-up is needed.

Jaundice or gastrointestinal bleeding needs prompt care

The United Kingdom's National Health Service says yellowing of the skin or whites of the eyes warrants urgent medical help and may occur with darker urine and paler stool. An AST-to-ALT ratio cannot identify the cause or make it safe to wait.

The same health service directs people who vomit blood and also have black stool, confusion, faintness, rapid or shallow breathing, cold, clammy and pale skin, or abdominal pain to emergency care. Its emergency number is specific to the United Kingdom; readers elsewhere need their local emergency service rather than another liver test.

Check raw values before calculating the ratio

The formula is AST divided by ALT. An AST of 40 U/L and ALT of 20 U/L give a ratio of 2.0, while an AST of 400 U/L and ALT of 200 U/L produce the same ratio. The calculation discards the tenfold difference between the two sets of raw values.

ACG's 2017 guideline says the degree of AST or ALT elevation helps direct the evaluation and recommends confirming an abnormal panel with repeat or clarifying tests. A clinician decides what needs confirmation from the complete report, symptoms and history rather than the ratio alone.

British Society of Gastroenterology guidance says abnormal liver blood tests should be interpreted with earlier results, medical history and the current clinical condition, and that the size of an abnormality does not necessarily show its clinical importance. The units and upper reference limits printed by the laboratory should stay with each result when values from different dates are compared.

Common patterns narrow questions but do not name a disease

ALT higher than AST

ACG lists viral hepatitis, fatty liver disease, alcohol-associated disease, autoimmune and inherited conditions, and medicine exposure among causes considered when AST or ALT is elevated. An ALT-predominant pattern does not by itself diagnose fatty liver disease or any other condition.

AST higher than ALT

The British guideline notes that AST is abundant in skeletal, cardiac and smooth muscle and asks clinicians to include muscle injury in the history. A higher AST can therefore reflect liver or non-liver tissue, and the alcohol and muscle history both matter.

ACG's 2024 guideline uses an AST-to-ALT ratio above 1.5 only among several criteria for probable alcohol-associated hepatitis, alongside recent jaundice, sustained heavy alcohol use, a defined AST and bilirubin pattern, and exclusion of other causes. A screening ratio of 2.0 does not supply those clinical conditions and cannot establish the diagnosis.

Both values inside the laboratory range

The British guideline says liver blood tests used in isolation are neither specific diagnostic tools nor specific exclusion tools. A ratio calculated from two in-range results does not override an existing follow-up plan based on chronic hepatitis, cirrhosis or another established risk.

Other results answer different questions

ACG describes AST and ALT as indirect markers of hepatobiliary disease rather than complete measures of liver function, and it distinguishes a hepatocellular pattern from a cholestatic pattern by comparing them with alkaline phosphatase. Bilirubin, albumin, alkaline phosphatase, gamma-glutamyl transferase (GGT) and a full blood count add information that the ratio omits.

For adults in the United Kingdom, the British guideline's standard cause screen includes abdominal ultrasound, hepatitis B and C tests, autoantibodies, immunoglobulins and iron studies. That package is not a worldwide order set; clinicians adapt testing to the presentation, local disease patterns and available services.

The ACG guideline also asks clinicians to review prescription drugs, nonprescription medicines, complementary products and dietary or herbal supplements when liver tests are abnormal. A ratio cannot prove that one product caused an injury, and a prescribed medicine should not be changed on that calculation alone.

Bring trends and exposure history to follow-up

The most useful record combines the full laboratory report with timing and exposure history. It lets a clinician compare the pattern across dates without losing the laboratory's reference limits or the circumstances around each blood draw.

  • The current report and available earlier reports show the exact AST and ALT values, units, dates and upper reference limits.
  • Other available results include bilirubin, albumin, alkaline phosphatase, GGT, the full blood count and any clotting tests already performed.
  • Earlier ultrasound reports and hepatitis B or C test results show what has already been assessed.
  • A complete exposure list covers alcohol, prescription and nonprescription medicines, herbal products and dietary supplements.
  • Notes on recent infection, muscle injury or strenuous activity place each blood draw in context.

The British guideline specifically calls for a history covering symptoms, other illnesses, prescribed and nonprescription products, alcohol, metabolic risks and muscle injury. Matching one ratio to an online label cannot replace that cause-based review.

Children and pregnancy require separate interpretation

A North American pediatric guideline uses child-specific ALT limits and says AST has not been independently validated as a screening test for pediatric fatty liver disease. A child's results need pediatric assessment rather than adult ratio rules.

The International Federation of Gynecology and Obstetrics (FIGO) provides pregnancy-specific laboratory ranges and recommends multidisciplinary care for new or pre-existing liver disease during pregnancy. Pregnant people with symptoms or an abnormal panel need obstetric assessment rather than self-interpretation against adult screening patterns.

People with chronic hepatitis, cirrhosis, metabolic disease or medicines that require monitoring also need an individualized plan. They should discuss changes in results, treatment or supplements with the clinician managing that condition.
